Well visits with weight checks at hospital discharge, One day after discharge, 2 weeks, one month, 2 months and 3 months:
4 Months – 24 Months
Well visits at 4, 6, 9, 12, 15, 18, and 24 months:
- Point-of-care testing and office visits as needed for urgent/sick visits included in the price of membership.
- Developmental screening.
- Monitoring of growth and nutrition.
- Guidance for what to expect at each developmental stage.
24 Months – 4 Years
Three well visits per year:
- Monitoring of growth and developmental milestones.
- School/Daycare forms completed.
- Guidance on Potty Training, Sleep Training, Behavioral concerns, etc.
4 Years – 21 Years
Two well child visits per year:
- Monitoring of growth and nutritional status.
- Monitoring and management of chronic conditions such as ADHD, asthma, seasonal allergies, autism, etc. Sports physical exams and form completion.

Yes, we offer family plans tailored for the well-being of your little ones.
